Nurse Practitioner-Led Clinics are a new primary health care delivery model in which Nurse Practitioners are the lead providers of primary health care. HF Connecting Health Nurse Practitioner-Led Clinic is set to open in the summer of 2013. Our clinic will provide service to those clients who do not have a physician or chooses to use the clinic for all of his or her primary care. Clients are offered a variety of services through an interdisciplinary team of Nurse Practitioner, Registered Nurse, Collaborating Physician, Dietitian, Social Worker and Health Promoter. Our clinic is focused on improving quality of care by integrating primary care with mental health services, health promotion, case management, disease prevention and chronic disease management.
The clinic is located in a busy plaza in Scarborough at Midland and Finch Avenue.

We are seeking three (3) Nurse Practitioners who enjoy working with an inter-professional health team. The Nurse Practitioner (NP) would be providing direct patient care in a community-based primary care clinic. The NP will report to the Lead Nurse Practitioner.
The ideal candidate will have advanced knowledge and skills in assessment, nursing, medical diagnosis, intervention, treatment management, drug therapy and referrals functions which are shared with collaborating physicians. Each NP will have a patient roster of approximately 800 patients and works with a team to provide family and client centered care.
Major Job Responsibilities:
 Provide clinical health assessments, diagnosis, nursing care and services to clients (individuals, families and groups) to promote a healthy lifestyle, to prevent and/or reduce incidence of disease and disability.
 Develop, implement and evaluate medical directives.
 Provide regular updates - reports regarding client care practices, trends, challenges and proposed solutions.
 Participate in the development, implementation and evaluation of the clinic’s quality improvement plan.
 Develop a plan of care based on patient needs, independently or in consultation with a collaborating physician and other members of the clinical team.
 Develop collaborative relationships with local area NPs and the regional NPAO chapter (for patient care needs, networking, resource sharing and to meet College of Nurses of Ontario Quality Assurance expectations).

 Liaise directly or coordinate another team member to link with community service organizations, agencies, hospitals to support the development of a primary health care network and reciprocal referral relationships.

Skills & Qualifications:
 Certification as a Primary Health Care NP in Ontario.

 3-5 years nursing experience in a community, hospital or primary care setting.
 Has knowledge of provincial health policy.

 Experience with electronic medical records system.
 Current Basic Cardiac Life Support (BCLS).
 Strong English written and verbal communication skills and proficiency in a second language. (Chinese, Korean or Vietnamese) are required.
 Mental health and acute care experience is considered an asset.
 Demonstrated ability to work in a multi-cultural setting.
 A satisfactory result of Vulnerable Sector Police Record Check is a requirement of this position.

London Family Health Team : The London Family Health Team is a group of interdisciplinary professionals working to improve the quality of life of the patients through prevention, identification, and management of disease.

Summary
The Nurse Practitioner is to work collaboratively across the London Family Health Team, including primary home care, primary care, specialized with the Elderly and Frail population. The Nurse Practitioner will support the care of the Elderly and Frail, the complex palliative patients, and the chronic but stable palliative patients for the physicians of the London Family Health Team.
The Nurse Practitioner will provide care connections across the health care system for Elderly and Frail, Palliative, and Dementia patients, while working in collaboration with the interdisciplinary team.
The Nurse Practitioner will share his/or her expertise and knowledge of health promotion, disease prevention, and chronic disease management across the lifespan of the LFHT patients.
The Nurse Practitioner conducts his or her duties in strict accordance with the Lead Physician directions, LFHT policies, safety procedures, and best practices.

Duties and Responsibilities
  •  Focus on educational of Patients and Care Givers of the following programs that are implemented within the LFHT Annual Operational Plan (e.g. Dementia, Palliative, Geriatric and Frail, etc)
  • To provide pain and symptom management for patients in their finial year of life.
  • To provide support and bereavement counseling for families.
  • Improve access to primary care for elderly.
  • Facilitate the ability for the elderly to remain in their homes.
  • Screen for cognitive difficulties including dementia.
  • Screen for osteoporosis and develop care plan for those at risk.
  • Screen from Depression in elderly with chronic disease.
  • Prevention of poly-pharmacy and reduction of medication.
  • Identification of patients with MCI, in order to stratify the diagnosis of dementia of severity and type.
  • Make home visits for complex palliative patients and their families, to conduct comprehensive clinical assessments and contribute to the development of comprehensive shared care plans in consultation with primary care physicians, CCAC case managers
  • Perform other duties within your legislated scope of practice as a NP, such as diagnosing, ordering and interpreting diagnostic tests, and prescribing pharmaceuticals.
  • Act as a resource to the CCAC Case manager in terms of clinical expertise in developing palliative care plans for complex patients of the LFHT.
  • Provide clinical advice and support for chronic palliative patients of the LFHT, and their families, as patient interact with home and community care, primary care, acute care, and specialist care.
  • Participate in systems planning and system integration to ensure a comprehensive and quality system of care for patients and their families.
  • Travel with Home Visits can accommodate up to 50% of the Nurse Practitioner work day.
  • Evening and weekend house calls could be required.
Skills / Abilities
  • Knowledge and proficiency in current evidenced based methods and practices of primary care delivery, with an emphasis on health promotion.
  • Leadership, organization, research, evaluation, time management, communication and interpersonal skills.
  • Proficiency in the use of the computer hardware and software, particularly in Microsoft Word, Excel, Outlook
  • Experience and knowledge of EMR.
  • Desire and ability to update and knowledge and skills through various means including technology-based opportunities, courses, workshops, and conferences.
  • Multi-tasking skills related to the delivery of efficient primary care including the ability to share information and teach while treating patients in a individual or group session.
  • Excellent Verbal and written communication skills.
  • Valid driver’s license and access to a reliable vehicle.
Professional Registration:
  • Current registration from the College of Nursing of Ontario as RN(EC)
  • Membership in the Registered Nurses Association of Ontario (preferred).

Personal Support Workers (PSW)

Qualifications:

As the ideal candidate you are a Registered Nurse with a Baccalaureate degree and/or Diploma in Nursing or evidence of working towards. You must be registered with the College of Nurses of Ontario. Experience in paediatric nursing is required. You have excellent interpersonal and communication skills and have demonstrated your ability to utilize your critical thinking, problem solving and teamwork abilities.
